Blood Pressure Review Form

Thank you for checking your blood pressure at home. This website has a good summary of how to take the most accurate readings using your home blood pressure monitor, click here.

If you have been asked to check your blood pressure at home to establish a new diagnosis of hypertension then please record 7 days of readings (check your blood pressure 2-3 times and record the lowest reading in both the morning and the evening for 7 consecutive days).

If you have an existing diagnosis of raised blood pressure then we only need 3 readings to confirm you are still on the correct treatment (check your blood pressure 3 times over the course of 2-3 days and record the lowest reading). You will only be contacted if your blood pressure is out of range and needs attention.
