Swine Flu The swine flu vaccination progam will continue only for patients in clinical at risk groups. Healthy children aged 6 months to 5 years will no longer be vaccinated following advice from the DoH.

Telephone Advice
If you want advice about test results or other matters and your doctor is unavailable when you call, you will be given a time to call back, e.g. at the beginning or end of surgery. Please try to avoid calling for results before 2.00 p.m.

The practice accepts new patients, resident in the SP11 postcode area (except Ludgershall) and co-residents of patients currently registered with the practice.

PreRegistration If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct. Please note that you should only complete this form if asked to do so by a Receptionist.

There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
