PATIENT’S RIGHTS AND RESPONSIBILITIES
Information held regarding patients is confidential and use of this is governed by The Data Protection Act, Access to Medical Records Act and Caldicott Guidelines. Information is shared with other Health Service Providers in patients’ interest (e.g. hospital referrals), with patients’ permission.
Requests for access by patients to their personal clinical records should be made in the first instance to Reception.
Patients have the right to chose with whom they book their consultation but will be offered an alternative if their chosen practitioner is fully booked or unavailable (Reception staff will advise).
In the event of being unable to keep appointments, patients are requested to cancel, in order that the consulting time is not wasted.
Patients aged 16-75 who have not been seen at the Surgery in the previous three years and patients aged 75 who have not been seen in the previous year, may book a health check with a nurse.
Patients can expect medical care and assistance according to need and in turn are requested to recognise difficulties that can sometimes occur, and maintain courtesy in any dealing with practice personnel.
Verbal or physical abuse to any person on practice premises justifies removal of registration with the practice.
The Practice operates a non-discrimination policy. Copies of Practice Procedures on the above topics are available on request.
